What is Enhanced Care Management?
Medi-Cal members may receive Enhanced Care Management (ECM) services provided by RecoverWell at no cost through their health plan. For patients who are not Medi-Cal members, RecoverWell provides the same hands-on care coordination support for transitions of care.

Care Managers are backed by Care Coordinators and clinical supervision, providing additional support to carry out the care plan and keep things moving.
Access Regardless of Insurance
Eligible Medi-Cal members may receive Enhanced Care Management (ECM) through their health plan. Grant funding allows RecoverWell to extend the same hands-on Care Management support to patients who are not Medi-Cal members.
What Does the RecoverWell Care Team do?
The RecoverWell Care Team does the heavy lifting so that care planners in hospitals and community care settings
can focus on counseling and supporting the patients in front of them. Some of the work we do:
- Review patient applications for missing information
- Provide education to patients and families who may need services beyond the current care setting.
- Benefits counseling
- Coordinate paperwork
- Find appropriate providers
- Submit applications
- Connect patients for intake interviews
- Help set care appointments
- Follow up with patients to provide ongoing resources
- Provide Reporting and updates
